Surgical Release Form

Print and complete this form to save time during your pet’s check-in for surgery. An Orchard Hills Animal Hospital staff member will review the form with you at the time of check-in.

Surgical Discharge Form

This is the form your pet will be sent home with after his or her surgery. An Orchard Hills Animal Hospital staff member will completely review the filled in form (we fill in the form) at the time of discharge.
